Request for Sharing and Celebrating an Event or Mission

Request for Fundraiser

This is the form for requesting a fund raiser for a current ministry area or for a new mission. This is not for emergency funding. Either fill out the Request for Fundraiser form directly on the website to submit it instantly via email, or print a copy and return it to Christine Nolan (Financial Secretary) in the Church Office.

This is the procedure for requesting church funds not approved in the current year budget. Request for Neighborhood Funds

This is the procedure for requesting neighborhood funds. The church has set aside special offerings in this fund to increase awareness of ministries in our church. Either fill out the Request for Neighborhood Funds form directly on the website to submit it instantly via email, or print a copy and return it to Christine Nolan (Financial Secretary) in the Church Office.
 

Request for Purchase/Payment

This is the procedure for requesting payment for purchases made after approval has been received. Please read these directions before making purchases for your ministry team. Then either fill out the Request for Purchase/Payment form directly on the website to submit it instantly via email, or print a copy and return it to Christine Nolan (Financial Secretary) in the Church Office.
 

Hospitality Assistance Request Form

This form is intended to be used by the Hospitality Team to help better serve ministry groups within the church when hospitality services are needed for meetings or events. Either fill out the Hospitality Assistance Request form directly on the website to submit it instantly via email, or print a copy and return it to the Church Office.
 

One Call Now & Constant Contact Opt-In Form

This is the form that gives MUMC permission to upload your contact information into the One Call Now and Constant Contact systems.
One Call Now is an automated phone and text-messaging system used primarily for church teams and committees, as well as the weekly Grapevine phone message. It’s also used if Sunday worship services are cancelled. It also supports email messages to team members.

Constant Contact is an email-only messaging system used to broadcast information about church events.
